Our financial counselors are here to guide you through finances and insurance. As a courtesy, we will consult with your insurance carrier to verify coverage, estimate any out-of-pocket costs, and if needed, recommend financial assistance programs, packages,and discounts that you may be eligible for to reduce your treatment expenses.

  • New York State Department of Health Infertility Demonstration Program

    The New York State Department of Health created a mandate in 2003 to provide financial subsidy grants so that more infertility patients could access the infertility and IVF treatments they need. Although there are certain clinical determinants, excluded services and coverage limitations to the grant program, the NY State IVF Grant Program has helped many patients achieve their dream of parenthood.

    Insured patients who meet eligibility criteria, and whose insurance does not cover IVF can apply for the New York State Department of Health Infertility Demonstration Program.

    In order to qualify for the New York State grant, patients must meet criteria as determined by the New York State Department of Health. This assessment is made through a new patient evaluation consultation and a clinical screening process conducted by your RMA of New York – Long Island doctor. Patients must provide proof of:

    • Age
    • New York state residency
    • Private health insurance as primary insurance (not Medicare)
    • Combined gross household income (determined by most recent tax returns) of under $195k annually. Qualified patients will pay a cost share amount. Out-of-pocket expenses are determined by NY State guidelines and are based on total household income level, ranging from 2.5% to 97.5% of the total costs.

  • Made possible by an endowment from generous benefactors, the Jade Foundation IVF Grant is offered exclusively at RMA of New York – Long Island and provides financial assistance to couples whose insurance does not cover IVF treatments. Qualified patients will pay a cost share amount, or out-of-pocket expense, based on total household income level. Funding through this grant program is allocated for basic services related to an IVF cycle, and excludes other types of treatments such as egg donation cycles, surrogacy cycles, elective egg freezing cycles, as well as genetic testing services, such as preimplantation genetic diagnosis or screening (PGT-A /PGT-M).

    Applicants must be Long Island, Queens, or Brooklyn residents less than 38 years of age, have documented infertility, and meet clinical and financial criteria. Additionally, applicants must have commercial medical insurance coverage which either does not cover their IVF treatments or for which they have exhausted their IVF benefits. Priority is given to first time parents and to those who have served or are currently serving in the U.S. military.

  • The Chick Mission Hope Grant program is available for cancer patients to ensure every young woman newly diagnosed with cancer has the option to preserve fertility through direct financial support, educational programs, and advocacy efforts.

  • An IVF package is created for each patient based on their personal family-building goals and the protocol they have agreed upon under the guidance of one of our physicians. Each package, therefore, is individualized.

    At RMA of New York – Long Island, IVF packages are typically batched into two categories: ‘Fresh Cycle’ and ‘Freeze All Cycle’. In a ‘Fresh Cycle’ patients undergo IVF with the intention of transferring fresh embryos that have not been frozen. This type of package includes monitoring (office visits, ultrasounds, and blood work), egg retrieval, conventional fertilization, and embryo transfer. ‘Freeze All Cycle’ patients undergo an egg retrieval and freeze their eggs or embryos, with the intention of transferring them later. This package includes monitoring (office visits, ultrasounds and bloodwork), egg retrieval, ICSI, cryopreservation, and embryo transfer at a later time.

    IVF packages at RMA of New York – Long Island do NOT include medications, anesthesia, or PGT, preimplantation genetic testing, of embryos. Cryopreservation, or freezing, of specimens, is included in a ‘Freeze All Cycle’ but not in a ‘Fresh Cycle’.

    A ‘Freeze All Cycle’ excludes the transfer. But there is a combined package of the ‘Freeze All Cycle’ plus a transfer that provides some cost savings. There is no timeframe as to when the transfer must occur.

Fertility Financing & Insurance FAQs

  • Coverage varies depending on your plan and the type of treatment. New York State has an infertility insurance mandate that requires many insurers to cover diagnostic testing and some forms of fertility treatment. IVF, IUI, and egg freezing may or may not be included. Our financial counseling team can review your benefits and explain what your specific plan covers.

  • RMA of New York – Long Island partners with trusted fertility financing providers, including Capex MD, ARC Fertility, and Future Family to offer loans and flexible payment plans, treatment bundles, and pharmacy discount programs. These options can help make fertility care more manageable for those paying out of pocket or with limited insurance coverage.

  • Yes, our partners are also partnering with the same loan companies as RMA of New York – Long Island.

  • If IVF is not covered by your insurance or only partially covered, there are still options. RMA of New York – Long Island offers access to loan programs, medication discount plans, and special packages that may reduce costs. Our counselors can also guide you through applying for pharmacy savings programs like Compassionate Care.

Some plans even recognize RMA NY Long Island as a Center of Excellence. We will verify your coverage and explain your fertility benefits before treatment begins.

  • Costs for IVF vary depending on your treatment plan, medication needs, and whether you choose services like PGT or ICSI. While RMA of New York – Long Island doesn’t publish exact pricing online, our financial team can provide you with a personalized cost estimate and explain available savings options.

  • Yes. Many fertility treatments, including IVF, IUI, fertility testing, and medications, are eligible expenses under Health Savings Accounts (HSAs) and Flexible Spending Accounts (FSAs). Our team can help you understand how to apply these funds toward your care.

  • Yes. RMALI offers an Active Duty Military and Veterans Discount Program, along with participation in the EMD Serono Compassionate Corps program, which provides savings on fertility medications for eligible veterans and their spouses.

  • RMA of New York – Long Island is currently offering the JADE IVF Grant and NYS DOH IVF Grant for qualified patients. Our financial services team will provide you with the grant application and will explain possible savings.

  • RMA of New York – Long Island partners with the LIVESTRONG Foundation and Chick Mission Foundation. The organizations support those whose cancer diagnosis may affect their ability to create or add to their families. They are dedicated to making life easier for cancer survivors- whether patients have just been diagnosed or further along on their journey. All patients will be asked to complete an eligibility screening before accessing an online application.

  • Yes. We partner with Embryo Options and they provide our patients with access to manage their storage online, including payments and discard forms.
